PROCEDURES FOR REGISTRATION


NEW REGISTRATION

Eligibility
A person who has successfully completed Diploma / Degree in Pharmacy or Pharm-D from an Institution approved by the Pharmacy Council of India will be eligible for registration as a ‘Registered Pharmacist’.

Documents to be produced
1. Original Diploma or Degree certificate along with a True copy*.duly attested by a gazetted officer
2. Original Course certificate issued from the Institution where the applicant has completed Degree / Diploma in Pharmacy along with a attested copy.
3. Original Final Year mark list and attested copy
4. Attested True copies* of
(i) S.S.L.C Certificate
(ii) Pre-Degree / Plus-2 Certificate
(iii) Diploma / Degree in Pharmacy
(iv) Practical Training certificate for Diploma Holders
(v) Mark lists - all the years
(vi) 2 Passport size photograph (one photo attested by a Gazetted officer on its reverse side stating
“This is the True photo of Mr. / Ms. _____________________________ S/o /D/o________________"

(Signature, Name, Designation and Office seal of the attesting officer)

(vii) Affidavit for B.Pharm holders stating that he / she has not undergone D.Pharm
(Viii) Declaration for B.Pharm holders stating that the original certificate will be produced within 6 months.

Fees
Fee of Rs.2600/- shall be paid either in cash counter in the office or by DD drawn in favor of the Registrar, Kerala State Pharmacy council payable at Trivandrum.

WHITE COAT CEREMONY
On receipt of application for Registration the date of white coat ceremony will be intimated immediately to the applicant. All the applicants should attend in the White Coat Ceremony by strictly wearing white over coat and should administer the Pharmacists’ oath. It is also intended to create an awareness about the laws and regulations governing the profession of Pharmacy, before receiving the license to practice the pharmacy profession. So the attendance in the white coat ceremony is compulsory for all the applicants who wish to assure in the Profession and Practice of Pharmacy in the state of Kerala. In case of any inconvenience to attend the white coat ceremony on the prescribed date, the matter should be intimated to the Council well in advance and a suitable posting should be obtained from the Registrar of the Kerala State Pharmacy Council.

TRANSFER OF REGISTRATION (RECIPROCAL REGISTRATION)

Eligibility
A person who is a Registered Pharmacist in any other state Pharmacy Council in India and who could produce the following documents, can transfer his/her registration to Kerala .

Documents to be produced

1. Original Registration Certificate issued by the state Pharmacy Council concerned along with a duly attested* true copy
2. Original Diploma or Degree certificate along with a duly attested* True copy
3. Original Course certificate issued from the institution from where the applicant has completed Diploma / Degree course
3. Attested* True copies of
(i) S.S.L.C Certificate
(ii) Pre-Degree/Plus2 Certificate
(iii) Diploma / Degree in Pharmacy
(iv) Practical Training certificate issued from an institution approved by
the Pharmacy Council, in the case of Diploma Holders
(v)Course certificate from the Institution where the applicant has undergone
Pharmacy course
(vi) True copies of mark lists and final year original mark list
(vii) Affidavit for B.Pharm holders
(viii) 2 Passport size photograph (one photo attested by a Gazetted officer on the
Outer side of the photo with the following statement.

"This is the True photo of Mr. / Ms. _____________________________ S/o / D/o _____________________________"

NOTE: (1) The Kerala State Pharmacy Council will grant Transfer of registration only after receiving No Objection Certificate (N.O.C) from other State Councils. In order to obtain N.O.C. the applicant should clear all the dues in Council Concerned. He / She should also surrender all the Certificates issued by the Council concerned.

(2)In case of Transfer from Tamilnadu Pharmacy Council , a DD for Rs.100/- taken in favour of Registrar, Tamilnadu Pharmacy Council should be submitted along with the application form.

(3) In case of Transfer from Maharashtra Pharmacy Council , a DD for Rs.500/- taken in favour of Registrar, Maharashtra Pharmacy Council should be submitted along with the application form.

White Coat Ceremony
All candidates who have applied for reciprocal registration before entering in the practice of pharmacy in the state, should acquire thorough knowledge in the Rules and regulations governing the Profession and Practice of Pharmacy. Hence, all the candidates should participate in the white coat ceremony and should administer Pharmacist’s oath. They should attend the CPE program in the ceremony and obtain a certificate of successful completion.

REGISTRATION RENEWAL

As per Pharmacy Act Section 34, all Pharmacist should renew their registration before 31st December every year. The State Council should publish the List of Pharmacist soon after the 1st day of April every year. The Pharmacist can renew their registration by paying Rs. 50/- as renewal fee. Those who have opted NRDS (Non-refundable Deposit Scheme) scheme should submit a request for renewal before 31st December in every year. They are exempted from paying renewal fee. KSPC grants a grace period from 1st January to 31st March for renewal of registration to those who could not make it before 31st December.
In defaulted cases the renewal application should be submitted in the prescribed format as given below.

REGISTRATION RENEWAL-LONG TERM[RESTORATION]
KSPC grants restoration of renewal of registration in genuine cases to those who could not renew their registration for more than 05 years. The approval of the council executive committee is mandatory for restoration of registration. Pharmacist”s shall apply to the Registrar by stating the reason for the lapse along with the documentary proof ,if any.

DUPLICATE CERTIFICATE
If the PRC is irrecoverably spoiled or lost a duplicate PRC can be obtained by remitting a fee of Rs.600/- . Such applicants should submit along with the application an affidavit from the 1st class Judicial Magistrate certifying that the PRC is irrecoverably lost and that the same will be surrendered in case it is recovered later. The duplicate PRC will be issued only affixing “DUPLICATE”.

ENTRY OF ADDITIONAL QUALIFICATIONS

Eligibility :-
A Registered Pharmacist on acquiring higher qualification related with Pharmacy and approved by Pharmacy council of India will be eligible to enter his/her additional qualification in the register of State Pharmacy Council and can obtain a certificate of additional qualification.

Documents to be produced
1. Duly attested* copy of Registration Certificate (PRC) issued by the State Pharmacy Council
2. Original Certificate of the newly acquired qualification for registration along with a True copy duly attested*
3. Course Certificate in original and attested copy
4. Final year original mark list of the newly acquired qualification.
5. Recent photograph by wearing white overcoat Duly attested* over leaf as given below.
"This is the True photo of Mr. / Ms. _____________________________ S/o / D/o _____________________________" (Signature, Name, Designation and Office seal of the attesting officer. * (by a gezetted officer).

Fees
Fee of Rs.250/-can be paid either in Cash in the office, DD or by MO
If paid by DD, the amount should be drawn in favor of the Registrar, Kerala State Pharmacy Council, payable at Trivandrum.

IDENTITY CARD
The Pharmacists who practice in the state of Kerala should wear ID card issued by the KSPC. By paying Rs.50/- Pharmacist can apply for ID card. For Duplicate ID Card application should be submitted in the prescribed application form. The validity of ID card will be 5 years. ID card can also be changed by applying through post by paying Rs.85/- as M.O. / DD in favour of the Registrar.

CHANGE OF CERTIFICATE

Procedure
The old Registration Certificate issued by the KSPC has been replaced with the new model high security certificate with hologram, as per the provisions in the Kerala Pharmacy Council Rules. Registered Pharmacists possessing the old format of Pharmacy Registration Certificate can change their certificate to new format. The Council will issue high security certificate and identity card within 20 days from the date of receipt of a perfect application along with the old PRC in original, if the applicant is otherwise eligible.
Replacement of PRC, ID Card and the postage expenses should be met by the applicant. DD for Rs 185/- drawn in favour of the Registrar, Kerala State Pharmacy Council should be enclosed along with the application.

VERIFICATION CERTIFICATE :
The applicant should apply in the prescribed application form for verification of certificate along with a request to the Registrar in white paper and copies of PRC, Diploma / Degree in Pharmacy. Fee of Rs.500/-.shall be paid either in cash in the office counter or by DD drawn in favor of Registrar, Kerala State Pharmacy Council, payable at Trivandrum. Verification certificate will be sent directly to the employer from the Council office.

GOOD STANDING CERTIFICATE :

Fee: Rs.1000/-.
The applicant should submit a request in white paper along with 2 copies of PRC and a copy of Diploma / Degree in Pharmacy. (Rs.80/- postage charge should be paid in addition to the prescribed fee if the GSC to be sent directly to the authority concerned)

TRAINING
The diploma holders should undergo 500 hrs / 3 months practical training as prescribed in the ER 1991 at the training centers approved by the Pharmacy Council. The Pharm D holders should undergo clinical training as prescribed by PCI.

TRAINING CENTERS
Government/Private hospitals can apply to the Registrar for training centre for the approval with all details available in the hospital.. Approval will be granted on the basis of the inspection report submitted by the inspector concerned. The authority to grant the approval rests with Executive Committee and PCI.

CHANGE OF PROFESSIONAL ADDRESS / RESIDENTIAL ADDRESS:
The Pharmacist should intimate the changes, if any; in their professional / residential address in writing to the Registrar.

TATKAL APPLICATIONS:-
At present all the applicants for all services have to wait for delivery of the certificates till their turn arrives as certificates are issued strictly on a priority basis. This creates problems for those who require certificates immediately for jobs abroad. Hence in order to obtain certificates immediately, a tatkal scheme is introduced.

Tatkal fees : Rs.1000/- + individual prescribed fees.

JOURNAL SUBSCRIPTION:-
Pharmacy Council Journal is the official publication of the Kerala State Pharmacy Council. It is published for the benefit of registered Pharmacists to update their knowledge as free of cost against the payment of annual postage charge.

PHARMACIST DESK REFERENCE:

Publication of Pharmacists Desk Reference – an exclusive reference book for the use of the Pharmacists pracitising in hospitals as well as in community pharmacies. It is a fact that Pharmacists have to face a lot of problems to answer the queries and to give proper guidance while dispensing medicines to patients. New drugs with severe actions and adverse reactions are launched into the market day by day. Patient counseling has a major role to reduce the agony of patients and make them aware of the storage conditions and safe usage of each and every item of medicine prescribed by Medical Practitioners. During the period of course, a pharmacist has to study about various aspects of medicaments, diseases, normal values, storage conditions etc. But it is not practical to remember all these things through out our life. Moreover new discoveries compel us to be alert on the latest information. KSPC has decided to take initiative to publish “Pharmacist Desk Reference” to help the Registered Pharmacist to satisfy their requirements and to serve the public in a better way.

LIST OF PHARMACISTS:-

The Register contains the list of all those pharmacists who have renewed their registration as on 31st March and that of the newly registered Pharmacists. It also contains a separate section with the list of removed Pharmacists due to death or kept under suspension or erased on account of punishment. It also contain the total strength of the registered Pharmacists as on 31st march. The Registration / renewal granted to the Pharmacists will be valid till the expiry of grace perion on 31st March.
Those Pharmacists who wish to practice in the state but are not enlisted in the register should obtain a certificate of renewal from the KSPC before engaging themselves in the practice of Pharmacy. This is applicable for those who come under the NRDS scheme also.

ENDOWMENT FOR PHARMACISTS:-

An endowment of Rs. 15000/- (Rupees Fifteen Thousand only) is given to the next kin of those Pharmacists who die before attaining the age of 55.

BEST PHARMACISTS AWARD:-
Best Pharmacist Awards are distributed every year to the Pharmacists engaged in teaching, manufacturing and dispensing of Drugs in Government and Private sectors as well as to those in other department like enforcement.

Cash awards, certificate and memento are distributed to the top scorers in B’Pharm and D’Pharm examinations conducted by the colleges in the State of Kerala.
